2. A non-refundable $50 application fee
3. A copy of most recent report card
4. A copy of Standardized Test Scores
5. Completed Academic Recommendation Form ONE (1) form for 1st - 6th Grade;  THREE (3) forms for 7th - 12th Grade
6.  A completed Character Reference Form (7th - 12th Grade ONLY)
7. A copy of complete transcript (7th - 12th Grade ONLY)

ADMISSION PROCEDURE

1. Admission Testing
2. After review of application materials, successful candidates will be invited for a formal interview.
3. A favorable decision by the Admission Committee

**Notification of acceptance will be sent after decision from Admission Committee has been made.
